Remove Language and LanguageCode structures
...
Use ISO language codes directly instead, which has the advantage that
new languages can be used in metadata without having up update the API
to support them. Closes #5 .

Due to an incompatibility between Sphinx 1.5.x and Breathe 4.4, pin the

Remove ICO file from repository
...
Instead generate it as a pre-build step. It can't be generated as part
of the CMake build process because MSVC changes the PATH so that another
convert.exe conflicts with imagemagick's.
