If the range spans a page boundary, the mmio access can be broke, fix it as
write emulation.
And we already get the guest physical address, so use it to read guest data
directly to avoid walking guest page table again

The idea is from Avi:
| We could cache the result of a miss in an spte by using a reserved bit, and
| checking the page fault error code (or seeing if we get an ept violation or
| ept misconfiguration), so if we get repeated mmio on a page, we don't need to

When the page fault is caused by mmio, we cache the info in the shadow page
table, and also set the reserved bits in the shadow page table, so if the mmio
is caused again, we can quickly identify it and emulate it directly
Searching mmio gfn in memslots is heavy since we need to walk all memeslots, it
can be reduced by this feature, and also avoid walking guest page table for
soft mmu.

Introduce some interfaces to modify spte as linux kernel does:
- mmu_spte_clear_track_bits, it set the spte from present to nonpresent, and
track the stat bits(accessed/dirty) of spte
- mmu_spte_clear_no_track, the same as mmu_spte_clear_track_bits except
tracking the stat bits
- mmu_spte_set, set spte from nonpresent to present
- mmu_spte_update, only update the stat bits
Now, it does not allowed to set spte from present to present, later, we can
drop the atomicly opration for X86_32 host, and it is the preparing work to
get spte on X86_32 host out of the mmu lock

If the page fault is caused by mmio, the gfn can not be found in memslots, and
'bad_pfn' is returned on gfn_to_hva path, so we can use 'bad_pfn' to identify
the mmio page fault.
And, to clarify the meaning of mmio pfn, we return fault page instead of bad
page when the gfn is not allowd to prefetch

Drop device_list_mutex for the reader side on clone_fs_devices and
btrfs_rm_device pathes since the fs_info->volume_mutex can ensure the device
list is not updated
btrfs_close_extra_devices is the initialized path, we can not add or remove
device at this time, so we can simply drop the mutex safely, like other
initialized function does(add_missing_dev, __find_device, __btrfs_open_devices
...).

On btrfs_congested_fn and __unplug_io_fn paths, we should hold
device_list_mutex to avoid remove/add device path to
update fs_devices->devices
On __btrfs_close_devices and btrfs_prepare_sprout paths, the devices in
fs_devices->devices or fs_devices->devices is updated, so we should hold
the mutex to avoid the reader side to reach them

merge_state can free the current state if it can be merged with the next node,
but in set_extent_bit(), after merge_state, we still use the current extent to

It doesn't allocate extent_state and check the result properly:
- in set_extent_bit, it doesn't allocate extent_state if the path is not
allowed wait
- in clear_extent_bit, it doesn't check the result after atomic-ly allocate,
we trigger BUG_ON() if it's fail
- if allocate fail, we trigger BUG_ON instead of returning -ENOMEM since
the return value of clear_extent_bit() is ignored by many callers

Currently we keep track of only two states: guest mode and host
mode. This patch adds an "exiting guest mode" state that tells
us that an IPI will happen soon, so unless we need to wait for the
IPI, we can avoid it completely.
Also
1: No need atomically to read/write ->mode in vcpu's thread
2: reorganize struct kvm_vcpu to make ->mode and ->requests
in the same cache line explicitly

It only allows to audit one guest in the system since:
- 'audit_point' is a glob variable
- mmu_audit_disable() is called in kvm_mmu_destroy(), so audit is disabled
after a guest exited
this patch fix those issues then allow to audit more guests at the same time

without holding vcpu->async_pf.lock, it will break if we are handling apfs
at this time.

There is a bugs in this function, we call gfn_to_pfn() and kvm_mmu_gva_to_gpa_read() in
atomic context(kvm_mmu_audit() is called under the spinlock(mmu_lock)'s protection).
This patch fix it by:
- introduce gfn_to_pfn_atomic instead of gfn_to_pfn
- get the mapping gfn from kvm_mmu_page_get_gfn()
And it adds 'notrap' ptes check in unsync/direct sps

Support prefetch ptes when intercept guest #PF, avoid to #PF by later
access
If we meet any failure in the prefetch path, we will exit it and
not try other ptes to avoid become heavy path

Introduce this function to get consecutive gfn's pages, it can reduce
gup's overload, used by later patch
