Sebastian Ott 135a8b4ce5 s390/cio: fix unbind of io_subchannel_driver
If the io_subchannel_driver is unbound from a subchannel it bluntly kills
all I/O on the subchannel and sets the ccw_device state to not operable
before deregistering the ccw_device. However, for online devices we should
set the device offline (disband path groups etc.) which does not happen if
the device is in not oper state.

Simply deregister the ccw device - ccw_device_remove is smart enough to set
the device offline properly. If everything fails call io_subchannel_quiesce
afterwards as a safeguard.

Sebastian Ott 4ed8773b50 KVM: s390: fix fallthrough annotation

Sebastian Ott 770b55c995 s390/cio: fix return code after missing interrupt
When a timeout occurs for users of ccw_device_start_timeout
we will stop the IO and call the drivers int handler with
the irb pointer set to ERR_PTR(-ETIMEDOUT). Sometimes
however we'd set the irb pointer to ERR_PTR(-EIO) which is
not intended. Just set the correct value in all codepaths.

Sebastian Ott f97a6b6c47 s390/cio: fix ccw_device_start_timeout API
There are cases a device driver can't start IO because the device is
currently in use by cio. In this case the device driver is notified
when the device is usable again.

Using ccw_device_start_timeout we would set the timeout (and change
an existing timeout) before we test for internal usage. Worst case
this could lead to an unexpected timer deletion.

Fix this by setting the timeout after we test for internal usage.

Sebastian Ott 366b77ae43 s390/eadm: fix CONFIG_BLOCK include dependency
Commit 2a842acab1 ("block: introduce new block status code type")
added blk_status_t usage to the eadm subchannel driver. However
blk_status_t is unknown when included via <linux/blkdev.h> for CONFIG_BLOCK=n.

Only include <linux/blk_types.h> since this is the only dependency eadm has.

This fixes build failures like below:

Sebastian Ott a5f1005517 s390/pci: handle insufficient resources during dma tlb flush
In a virtualized setup lazy flushing can lead to the hypervisor
running out of resources when lots of guest pages need to be
pinned. In this situation simply trigger a global flush to give
the hypervisor a chance to free some of these resources.

Sebastian Ott 30e8eb8671 s390/qdio: sanitize put_indicator
qdio maintains an array of struct indicator_t. put_indicator takes a pointer
to a member of a struct indicator_t within that array, calculates the index,
and uses the array and the index to get the struct indicator_t.

Simply use the pointer directly.

Although the pointer happens to point to the first member of that struct
use the container_of macro.

Sebastian Ott 0dcd91a9e6 s390/debug: only write data once
debug_event_common memsets the active debug entry with zeros to
prevent stale data leakage. This is overwritten with the actual
debug data in the next step. Only write zeros to that part of the
debug entry that's not used by new debug data.

Micro benchmarks show a 2-10% reduction of cpu cycles with this
approach.

Sebastian Ott 94158e544f s390/debug: improve debug_event
debug_event currently truncates the data if used with a size larger than
the buf_size of the debug feature. For lots of callers of this function,
wrappers have been implemented that loop until all data is handled.

Move that functionality into debug_event_common and get rid of the wrappers.

Sebastian Ott cb09b356cd s390/cmf: avg_utilization
All (but one) cmf related sysfs attributes have been converted to read
directly from the measurement block using cmf_read. This is not
possible for the avg_utilization attribute since this is an aggregation
of several values for which cmf_read only returns average values.

Move the computation of the utilization value to the cmf_read interface
such that it can use the raw data.

Sebastian Ott d4d287e81f s390/cmf: read from hw buffer
To ensure data consistency when reading from the channel measurement
block we wait for the subchannel to become idle and copy the whole
block. This is unnecessary when all we do is export the individual
values via sysfs. Read the values for sysfs export directly from
the measurement block.

cmf_copy_block tries to ensure data consistency by copying the
channel measurement block twice and comparing the data. This was
needed on very old machines only. Nowadays we guarantee
consistency by copying the data when the channel is idle.

Sebastian Ott adc69b4d76 s390/cmf: set_schib_wait add timeout
When enabling channel measurement fails with a busy condition we wait
for the next interrupt to arrive before we retry the operation. For
devices which usually don't create interrupts we wait forever.

Although the waiting is done interruptible that behavior is not
expected and confused some users. Abort the operation after a 10s
timeout.

Sebastian Ott 55fb734757 s390/cio: recover from bad paths
In some situations we don't receive notification from firmware that
a previously unusable channelpath is usable again.

Schedule recovery for devices that return from path verification
without using all potentially usable paths. The recovery thread will
periodically trigger a path verification on the affected devices.

Sebastian Ott a3c1a2194a s390/scm: use common completion path
Since commit caf7df1227 ("block: remove the errors field from struct request")
rq->errors can't be (mis)used by block device drivers to store the error
condition for usage during async completion. Because of that I simply used
async completion only for the non-error paths.

This patch places the error within the private data of struct request and
uses async completion for all paths again.

Sebastian Ott 312e8462ab s390/pci: recognize name clashes with uids
When uid checking is enabled firmware guarantees uniqueness of the uids
and we use them for device enumeration. Tests have shown that uid checking
can be toggled at runtime. This is unfortunate since it can lead to name
clashes.

Recognize these name clashes by allocating bits in zpci_domain even for
firmware provided ids.

Sebastian Ott 01553d9a2b s390/pci: fix handling of PEC 306
In contrast to other hotplug events PEC 0x306 isn't about a single
but multiple devices. Also there's no information on what happened
to these devices. We correctly handled hotplug that way but failed
to handle hot-unplug. This patch addresses that and implements
hot-unplug of multiple devices via PEC 306.

Sebastian Ott 623bd44d3f s390/pci: improve pci hotplug
PCI hotplug events basically notify about the new state of a
function. Unfortunately some hypervisors implement hotplug
events in a way where it is not clear what the new state of
the function should be.

Use clp_get_state to find the current state of the function
and handle accordingly.

Code handling pci hotplug needs to determine the configuration

Sebastian Ott 4e5bd7803b s390/pci: improve error handling during fmb (de)registration
Cleanup in zpci_fmb_enable_device when fmb registration fails. Also
don't free the fmb when deregistration fails in zpci_fmb_disable_device
but handle error situations when a function was hot-unplugged.

Also remove the mod_pci helper since it is no longer used.

Sebastian Ott 7257083491 s390/pci: improve unreg_ioat error handling
DMA tables are freed in zpci_dma_exit_device regardless of the return
code of zpci_unregister_ioat. This could lead to a use after free. On
the other hand during function hot-unplug, zpci_unregister_ioat will
always fail since the function is already gone.

So let zpci_unregister_ioat report success when the function is gone
but don't cleanup the dma table when a function could still have it
in access.

Sebastian Ott 4dfbd3efe3 s390/pci: improve error handling during interrupt deregistration
When we ask a function to stop creating interrupts this may fail
due to the function being already gone (e.g. after hot-unplug).

Consequently we don't free associated resources like summary bits
and bit vectors used for irq processing. This could lead to
situations where we ran out of these resources and fail to setup
new interrupts.

The fix is to just ignore the errors in cases where we can be
sure no new interrupts are generated.

Users complained that they are hitting the limit of 64 functions (some

Sebastian Ott 98cc43ab6b s390/cio: clarify cssid usage
Currently the cssid in various structures is used as the id of
the respective channel subsystem. Sometimes however we call the
index in the channel_subsystems array cssid. In some places the
id is even used as the index.

Provide a new define MAX_CSS_IDX and use it where appropriate.
In addition to that provide a dummy function to find a channel
subsystem by its id and a macro to iterate over the channel
subsystems.

Sebastian Ott 17069d32a3 IB/core: fix unmap_sg argument
__ib_umem_release calls dma_unmap_sg with a different number of
sg_entries than ib_umem_get uses for dma_map_sg. This might cause
trouble for implementations that merge sglist entries and results
in the following dma debug complaint:

DMA-API: device driver frees DMA sg list with different entry
         count [map count=2] [unmap count=1]

Fix it by using the correct value.

Sebastian Ott 4f5359e94b s390/pci_dma: make lazy flush independent from the tlb_refresh bit
We have 2 strategies to reduce the number of RPCIT instructions:
* A HW feature indicated via the tlb_refresh bit allows us to omit RPCIT for
  invalid -> valid translation-table entry updates.
* With "lazy flush" we omit RPCIT for valid -> invalid updates until we run
  out of dma addresses. When we have to reuse dma addresses we issue a global
  tlb flush using only one RPCIT instruction.

Currently lazy flushing depends on tlb_refresh. Since there is no technical
reason for this remove this dependency.

Sebastian Ott 179a98cba1 s390/cio: don't register chpids in reserved state
During IPL we register all chpids that are not in the unrecognized
state. This includes chpids that are not usable and chpids for which
the state could not be obtained.

Change that to only register chpids in the configured (usable) or
standby (usable after a configure operation) state. All other chpids
could only be made available by external control for which we would
receive machine checks.

Sebastian Ott d53c51f261 s390/cio: fix accidental interrupt enabling during resume
Since commit 9f3d6d7 chsc_get_channel_measurement_chars is called with
interrupts disabled during resume from hibernate. Since this function
used spin_unlock_irq, interrupts have been enabled accidentally. Fix
this by using the irqsave variant.

Since we can't guarantee the IRQ-enablement state for all (future/
external) callers, change the locking in related functions to prevent
similar bugs in the future.

Sebastian Ott 13decfb438 misc/genwqe: ensure zero initialization
Genwqe uses dma_alloc_coherent and depends on zero initialized memory. On
one occasion it ueses an explicit memset on others it uses un-initialized
memory.

This bug was covered because some archs actually return zero initialized
memory when using dma_alloc_coherent but this is by no means guaranteed.
Simply switch to dma_zalloc_coherent.

Sebastian Ott 13954fd691 s390/pci_dma: improve lazy flush for unmap
Lazy unmap (defer tlb flush after unmap until dma address reuse) can
greatly reduce the number of RPCIT instructions in the best case. In
reality we are often far away from the best case scenario because our
implementation suffers from the following problem:

To create dma addresses we maintain an iommu bitmap and a pointer into
that bitmap to mark the start of the next search. That pointer moves from
the start to the end of that bitmap and we issue a global tlb flush
once that pointer wraps around. To prevent address reuse before we issue
the tlb flush we even have to move the next pointer during unmaps - when
clearing a bit > next. This could lead to a situation where we only use
the rear part of that bitmap and issue more tlb flushes than expected.

To fix this we no longer clear bits during unmap but maintain a 2nd
bitmap which we use to mark addresses that can't be reused until we issue
the global tlb flush after wrap around.

Split dma_update_trans into __dma_update_trans which handles updating
the dma translation tables and __dma_purge_tlb which takes care of
purging associated entries in the dma translation lookaside buffer.

The map_sg API makes use of this split approach by calling
__dma_update_trans once per physically contiguous address range but
__dma_purge_tlb only once per dma contiguous address range.

This results in less invocations of the expensive RPCIT instruction
when using map_sg.

Sebastian Ott f003a1f182 lib/iommu-helper: skip to next segment
When a large enough area in the iommu bitmap is found but would span a
boundary we continue the search starting from the next bit position.
For large allocations this can lead to several useless invocations of
bitmap_find_next_zero_area() and iommu_is_span_boundary().

Continue the search from the start of the next segment (which is the
next bit position such that we'll not cross the same segment boundary
again).

Sebastian Ott 4475aeb8b7 s390/cio: fix premature wakeup during chp configure
We store requests for channel path configure operations in an array but
maintain an additional cfg_busy variable (indicating if we have requests
stored in said array). When 2 tasks request a channel path configure
operation cfg_busy could be set to 0 even if we still have unprocessed
requests. This would lead to the second task being woken up although its
request was not processed yet.

Fix that by getting rid of cfg_busy and use the chp_cfg_task array
in the wake up condition.

Sebastian Ott 837c522055 s390/cio: convert cfg_lock mutex to spinlock
cfg_lock is never held long and we don't want to sleep while the
lock is being held. Thus it can be converted to a simple spinlock.
In addition we can now use the lock during the evaluation of a
wake_up condition.

Sebastian Ott f9773768f1 s390/chsc: sanitize fmt check for chp_desc determination
When fetching channel path descriptors we've only evaluated the rfmt
parameter which could lead us to trigger the chsc even though the
machine doesn't support the specific format or to not trigger the
chsc and report a failure to userspace even though the machine would've
supported it.

Improve these checks and change the parameters of the in-kernel
user to be less confusing.

Sebastian Ott 0b60137377 s390/cio: make fmt1 channel path descriptor optional
Not all machines / hypervisors support the chsc commands to fetch
the fmt1 descriptor. When these commands fail the channel path would
currently not be available to linux.

Since users of these descriptors can already deal with invalid data
make fetching it optional. The only data that is mandatory for us is
the fmt0 channel path descriptor.

Also make the return code for missing facilities in
chsc_get_channel_measurement_chars consistent to other functions.

Sebastian Ott 0f5d050cea s390/cio: allow to reset channel measurement block
Prior to commit 1bc6664bdf a call to
enable_cmf for a device for which channel measurement was already
enabled resulted in a reset of the measurement data.

What looked like bugs at the time (a 2nd allocation was triggered
but failed, reset was called regardless of previous failures, and
errors have not been reported to userspace) was actually something
at least one userspace tool depended on. Restore that behavior in
a sane way.

Sebastian Ott 8ee2db3cf1 s390/pci: ensure to not cross a dma segment boundary
When we use the iommu_area_alloc helper to get dma addresses
we specify the boundary_size parameter but not the offset (called
shift in this context).

As long as the offset (start_dma) is a multiple of the boundary
we're ok (on current machines start_dma always seems to be 4GB).

Don't leave this to chance and specify the offset for iommu_area_alloc.

Sebastian Ott bb98f396f1 s390: use SPARSE_IRQ
Use dynamically allocated irq descriptors on s390 which allows
us to get rid of the s390 specific config option PCI_NR_MSI and
exploit more MSI interrupts. Also the size of the kernel image
is reduced by 131K (using performance_defconfig).

Sebastian Ott dba599091c s390/pci: fix use after free in dma_init
After a failure during registration of the dma_table (because of the
function being in error state) we free its memory but don't reset the
associated pointer to zero.

When we then receive a notification from firmware (about the function
being in error state) we'll try to walk and free the dma_table again.

Fix this by resetting the dma_table pointer. In addition to that make
sure that we free the iommu_bitmap when appropriate.

Sebastian Ott 3fa7ee8844 s390/sclp: event type macro cleanup
Sort the sclp event type defines and use a macro to create the
corresponding event type masks. In addition to that one unused
type/mask pair is removed and another previously unused define
is used now (it was probably unused/unknown because it didn't
follow the EVTYP_X EVTYP_X_MASK convention).
