Only create_module existed. The other kinds in KIND had no
constructor, so nothing could build those nodes.

create_module stored kind as the string "module". The KIND table and
the constructor comment both use the integer ast.KIND.MODULE.

create_location_table listed column_number without a key, so Lua stored
it as array index 1 and location.column_number was nil. The function
comment already documents the keyed field.
