Update kryptopyr's Patch Hub (loot/skyrimse#2578)

* Move plugin
  * Group with plugins from same page.
  * No longer available from KPH FOMOD.
  * Now hosted on HA page.

* Add plugin
  * Group with plugins from same page.

* Remove `alreadyInX` message
  * No longer the case with latest version of CCOR patch from HA page.

* Add `patchOutdated` message
  * Patch is severely outdated & has many unresolved references with
    latest version of HA.

* Remove plugin
  * No more metadata.
