Registers are always 32 bit aligned. R_MAX is not the maximum
register address, it is the maximum register number. The memory
size can be determined by 4 * R_MAX.
Currently every register with an offset bigger than 0x40 will be
ignored, because the memory size is set wrong. This effects the
MCTRL register and makes it useless. This commit restores the
correct behaviour.

The 'xlnx_csu_dma_src_notify' method has a 4k byte array used for
copying DMA data. Skip the automatic zero-init of this array to
eliminate the performance overhead in the I/O hot path.
The 'buf' array will be fully initialized when data is copied.

In hw/arm and include/hw/arm, some source files for the OMAP SoC
and the sx1 boards that are our only remaining OMAP boards still
have hard-coded tabs (almost entirely used for the indent on
inline comments, not for actual code indent).
Replace the tabs with spaces using vim :retab. I used 4 spaces
except in some defines and comments where I tried to put
everything aligned in the same column for better readability.
This commit is a purely whitespace-only change.

In axidma_class_init() we accidentally used a comma at the end of
a statement rather than a semicolon. This has no ill effects, but
it's obviously not intended and it means that Coccinelle scripts
for instance will fail to match on the two statements. Use a
semicolon instead.

Loading a description from memory may cause a bus-error. In this
case, the DMA should stop working, set the error flag, and return
the failure value.
When calling the loading a description function, it should be noticed
that the function may return a failure value. Breaking the loop in this
case is one of the possible ways to handle it.

In fdf029762f we factored out the handling of reading and writing
DMA descriptors from guest memory. Unfortunately we accidentally
made the descriptor-read read the descriptor into the address of the
buffer rather than into the buffer, because we didn't notice we
needed to update the arguments to the dma_memory_read() call. Before
the refactoring, "&desc" is the address of a local struct DPDMADescriptor
variable in xlnx_dpdma_start_operation(), which is the correct target
for the guest-memory-read. But after the refactoring 'desc' is the
"DPDMADescriptor *desc" argument to the new function, and so it is
already an address.
This bug is an overrun of a stack variable, since a pointer is at
most 8 bytes long and we try to read 64 bytes, as well as being
incorrect behaviour.
Pass 'desc' rather than '&desc' as the dma_memory_read() argument
to fix this.
(The same bug is not present in xlnx_dpdma_write_descriptor(),
because there we are writing the descriptor from a local struct
variable "DPDMADescriptor tmp_desc" and so passing &tmp_desc to
dma_memory_write() is correct.)

Add xlnx_dpdma_read_descriptor() and
xlnx_dpdma_write_descriptor() functions.
xlnx_dpdma_read_descriptor() combines reading a
descriptor from desc_addr by calling dma_memory_read()
and swapping the desc fields from guest memory order
to host memory order. xlnx_dpdma_write_descriptor()
performs similar actions when writing a descriptor.
Found by Linux Verification Center (linuxtesting.org) with SVACE.

The DMA descriptor structures for this device have
a set of "address extension" fields which extend the 32
bit source addresses with an extra 16 bits to give a
48 bit address:

However, we misimplemented this address extension in several ways:
* we only extracted 12 bits of the extension fields, not 16
* we didn't shift the extension field up far enough
* we accidentally did the shift as 32-bit arithmetic, which
meant that we would have an overflow instead of setting
bits [47:32] of the resulting 64-bit address
Add a type cast and use extract64() instead of extract32()
to avoid integer overflow on addition. Fix bit fields
extraction according to documentation.
Found by Linux Verification Center (linuxtesting.org) with SVACE.
