e58d9a62b9 bug 931399 - shutdown DocAccessible's when the related docshell is destroyed r=bz 
When we get to PresShell::Destroy the pres shell and document have already
nulled out their poitners to the docshell which DocAccessible::Shutdown()
needs to remove the document from the command observer.

360f41735f Bug 811301. Part 3: Cleanup presshell code now that we don't do special state save/restore for the root scroll frame. r=mats 
Scrolling is never synchronous now, so we don't need to block scripts
or reflows when restoring the scroll position.
