The cargo-llvm-cov + grcov coverage report pipeline has been broken for a while and is no longer used. Drop the GitHub Actions workflow, the `cargo xtask cov` subcommands and module, the pinned tool versions, and the related `.gitignore` and AGENTS.md entries.
