Tobias HuschleandVasily Gorbik cf8771ca4c mm/page_table_check: Pass mm_struct to pxx_user_accessible_page()
Unlike other architectures, s390 does not have means to
distinguish kernel vs user page table entries - neither
an entry itself, nor the address could be used for that.
It is only the mm_struct that indicates whether an entry
in question is mapped to a user space. So pass mm_struct
to pxx_user_accessible_page() callbacks.

Rohan McLureandAndrew Morton d79f9c9cf7 mm: provide address parameter to p{te,md,ud}_user_accessible_page()
On several powerpc platforms, a page table entry may not imply whether the
relevant mapping is for userspace or kernelspace.  Instead, such platforms
infer this by the address which is being accessed.

Add an additional address argument to each of these routines in order to
provide support for page table check on powerpc.

This reverts commit aa232204c4 ("mm/page_table_check: remove unused
parameter in [__]page_table_check_pte_clear").

Reinstate previously unused parameters for the purpose of supporting
powerpc platforms, as many do not encode user/kernel ownership of the page
in the pte, but instead in the address of the access.

Rohan McLureandAndrew Morton 0a5ae44831 mm/page_table_check: provide addr parameter to page_table_check_ptes_set()
To provide support for powerpc platforms, provide an addr parameter to the
__page_table_check_ptes_set() and page_table_check_ptes_set() routines. 
This parameter is needed on some powerpc platforms which do not encode
whether a mapping is for user or kernel in the pte.  On such platforms,
this can be inferred from the addr parameter.

Lorenzo StoakesandAndrew Morton 0ac881efe1 mm: replace pmd_to_swp_entry() with softleaf_from_pmd()
Introduce softleaf_from_pmd() to do the equivalent operation for PMDs that
softleaf_from_pte() fulfils, and cascade changes through code base
accordingly, introducing helpers as necessary.

We are then able to eliminate pmd_to_swp_entry(),
is_pmd_migration_entry(), is_pmd_device_private_entry() and
is_pmd_non_present_folio_entry().

This further establishes the use of leaf operations throughout the code
base and further establishes the foundations for eliminating
is_swap_pmd().

No functional change intended.

Lorenzo StoakesandAndrew Morton aa62204cb6 mm: avoid unnecessary use of is_swap_pmd()
PMD 'non-swap' swap entries are currently used for PMD-level migration
entries and device private entries.

To add to the confusion in this terminology we use is_swap_pmd() in an
inconsistent way similar to how is_swap_pte() was being used - sometimes
adopting the convention that !pmd_none(), !pmd_present() implies PMD 'swap'
entry, sometimes not.

This patch handles the low-hanging fruit of cases where we can simply
substitute other predicates for is_swap_pmd().

No functional change intended.

Lorenzo StoakesandAndrew Morton fb888710e2 mm: avoid unnecessary uses of is_swap_pte()
There's an established convention in the kernel that we treat PTEs as
containing swap entries (and the unfortunately named non-swap swap
entries) should they be neither empty (i.e.  pte_none() evaluating true)
nor present (i.e.  pte_present() evaluating true).

However, there is some inconsistency in how this is applied, as we also
have the is_swap_pte() helper which explicitly performs this check:

	/* check whether a pte points to a swap entry */
	static inline int is_swap_pte(pte_t pte)
	{
		return !pte_none(pte) && !pte_present(pte);
	}

As this represents a predicate, and it's logical to assume that in order
to establish that a PTE entry can correctly be manipulated as a
swap/non-swap entry, this predicate seems as if it must first be checked.

But we instead, we far more often utilise the established convention of
checking pte_none() / pte_present() before operating on entries as if they
were swap/non-swap.

This patch works towards correcting this inconsistency by removing all
uses of is_swap_pte() where we are already in a position where we perform
pte_none()/pte_present() checks anyway or otherwise it is clearly logical
to do so.

We also take advantage of the fact that pte_swp_uffd_wp() is only set on
swap entries.

Additionally, update comments referencing to is_swap_pte() and
non_swap_entry().

No functional change intended.

Ryan RobertsandWill Deacon 91e40668e7 mm/page_table_check: Batch-check pmds/puds just like ptes
Convert page_table_check_p[mu]d_set(...) to
page_table_check_p[mu]ds_set(..., nr) to allow checking a contiguous set
of pmds/puds in single batch. We retain page_table_check_p[mu]d_set(...)
as macros that call new batch functions with nr=1 for compatibility.

arm64 is about to reorganise its pte/pmd/pud helpers to reuse more code
and to allow the implementation for huge_pte to more efficiently set
ptes/pmds/puds in batches. We need these batch-helpers to make the
refactoring possible.

Peter XuandAndrew Morton 8bb592c2ec mm/page_table_check: fix crash on ZONE_DEVICE
Not all pages may apply to pgtable check.  One example is ZONE_DEVICE
pages: they map PFNs directly, and they don't allocate page_ext at all
even if there's struct page around.  One may reference
devm_memremap_pages().

When both ZONE_DEVICE and page-table-check enabled, then try to map some
dax memories, one can trigger kernel bug constantly now when the kernel
was trying to inject some pfn maps on the dax device:

 kernel BUG at mm/page_table_check.c:55!

While it's pretty legal to use set_pxx_at() for ZONE_DEVICE pages for page
fault resolutions, skip all the checks if page_ext doesn't even exist in
pgtable checker, which applies to ZONE_DEVICE but maybe more.

Peter XuandAndrew Morton 8430557fc5 mm/page_table_check: support userfault wr-protect entries
Allow page_table_check hooks to check over userfaultfd wr-protect criteria
upon pgtable updates.  The rule is no co-existance allowed for any
writable flag against userfault wr-protect flag.

This should be better than c2da319c2e, where we used to only sanitize such
issues during a pgtable walk, but when hitting such issue we don't have a
good chance to know where does that writable bit came from [1], so that
even the pgtable walk exposes a kernel bug (which is still helpful on
triaging) but not easy to track and debug.

Now we switch to track the source.  It's much easier too with the recent
introduction of page table check.

There are some limitations with using the page table check here for
userfaultfd wr-protect purpose:

  - It is only enabled with explicit enablement of page table check configs
  and/or boot parameters, but should be good enough to track at least
  syzbot issues, as syzbot should enable PAGE_TABLE_CHECK[_ENFORCED] for
  x86 [1].  We used to have DEBUG_VM but it's now off for most distros,
  while distros also normally not enable PAGE_TABLE_CHECK[_ENFORCED], which
  is similar.

  - It conditionally works with the ptep_modify_prot API.  It will be
  bypassed when e.g. XEN PV is enabled, however still work for most of the
  rest scenarios, which should be the common cases so should be good
  enough.

  - Hugetlb check is a bit hairy, as the page table check cannot identify
  hugetlb pte or normal pte via trapping at set_pte_at(), because of the
  current design where hugetlb maps every layers to pte_t... For example,
  the default set_huge_pte_at() can invoke set_pte_at() directly and lose
  the hugetlb context, treating it the same as a normal pte_t. So far it's
  fine because we have huge_pte_uffd_wp() always equals to pte_uffd_wp() as
  long as supported (x86 only).  It'll be a bigger problem when we'll
  define _PAGE_UFFD_WP differently at various pgtable levels, because then
  one huge_pte_uffd_wp() per-arch will stop making sense first.. as of now
  we can leave this for later too.

This patch also removes commit c2da319c2e altogether, as we have something
better now.
