The function kgdb_wait() was folded into the static function
kgdb_cpu_enter() by commit 62fae31219 ("kgdb: eliminate
kgdb_wait(), all cpus enter the same way"). Update the four stale
references accordingly:
- include/linux/kgdb.h and arch/x86/kernel/kgdb.c: the
kgdb_roundup_cpus() kdoc describes what other CPUs are rounded up
to call. Because kgdb_cpu_enter() is static, the correct public
entry point is kgdb_handle_exception(); also fix a pre-existing
grammar error ("get them be" -> "get them into") and reflow the
text.
- kernel/debug/debug_core.c: replace with the generic description
"the debug trap handler", since the actual entry path is
architecture-specific.
- kernel/debug/gdbstub.c: kgdb_cpu_enter() is correct here (it
describes internal state, not a call target); add the missing
parentheses.

Remove <linux/hex.h> from <linux/kernel.h> and update all users/callers of
hex.h interfaces to directly #include <linux/hex.h> as part of the process
of putting kernel.h on a diet.
Removing hex.h from kernel.h means that 36K C source files don't have to
pay the price of parsing hex.h for the roughly 120 C source files that
need it.
This change has been build-tested with allmodconfig on most ARCHes. Also,
all users/callers of <linux/hex.h> in the entire source tree have been
updated if needed (if not already #included).

During debug trap execution we expect dbg_deactivate_sw_breakpoints()
to be paired with an dbg_activate_sw_breakpoint(). Currently although
the calls are paired correctly they are needlessly smeared across three
different functions. Worse this also results in code to drive polled I/O
being called with breakpoints activated which, in turn, needlessly
increases the set of functions that will recursively trap if breakpointed.
Fix this by moving the activation of breakpoints into the debug core.

This kills using the do_each_thread/while_each_thread combo to
iterate all threads and uses for_each_process_thread() instead,
maintaining semantics. while_each_thread() is ultimately racy
and deprecated; although in this particular case there is no
concurrency so it doesn't matter. Still lets trivially get rid
of two more users.

The XML packet could be supported by required architecture if the
architecture defines CONFIG_HAVE_ARCH_KGDB_QXFER_PKT and implement its own
kgdb_arch_handle_qxfer_pkt(). Except for the kgdb_arch_handle_qxfer_pkt(),
the architecture also needs to record the feature supported by gdb stub
into the kgdb_arch_gdb_stub_feature, and these features will be reported
to host gdb when gdb stub receives the qSupported packet.

We are going to split <linux/sched/signal.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/signal.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.

There's no reason kgdb.h itself needs to include the 8250 serial port
header file. So push it down to the _very_ limited number of individual
drivers that need the values in that file, and fix up the places where
people really wanted serial_core.h and platform_device.h.

The gdbstub and kdb should get detached if the system is rebooting.
Calling gdbstub_exit() will set the proper debug core state and send a
message to any debugger that is connected to correctly detach.
An attached debugger will receive the exit code from
include/linux/reboot.h based on SYS_HALT, SYS_REBOOT, etc...

When switching from kdb mode to kgdb mode packets were getting lost
depending on the size of the fifo queue of the serial chip. When gdb
initially connects if it is in kdb mode it should entirely send any
character buffer over to the gdbstub when switching connections.
Previously kdb was zero'ing out the character buffer and this could
lead to gdb failing to connect at all, or a lengthy pause could occur
on the initial connect.

Notify GDB of the machine halting, rebooting or powering off by sending it an
exited command (remote protocol command 'W'). This is done by calling:
void gdbstub_exit(int status)
from the arch's machine_{halt,restart,power_off}() functions with an
appropriate exit status to be reported to GDB.
