When the console out buffer is filled, __write_console() will return 0
as it cannot send any data. domU_write_console() will then spin in
`while (len)` as len doesn't decrement until xenconsoled attaches. This
would block a domU and nullify the parallelism of Hyperlaunch until dom0
userspace starts xenconsoled, which empties the buffer.
Xen 4.21 added a connection field to the xen console page. This is set
to XENCONSOLE_DISCONNECTED (1) when a domain is built, and xenconsoled
will set it to XENCONSOLE_CONNECTED (0) when it connects.
Update the hvc_xen driver to check the field. When the field is
disconnected, drop the write with -ENOTCONN. We only drop the write
when the field is XENCONSOLE_DISCONNECTED (1) to try for maximum
compatibility. The Xen toolstack has historically zero initialized the
console, so it should see XENCONSOLE_CONNECTED (0) by default. If an
implemenation used uninitialized memory, only checking for
XENCONSOLE_DISCONNECTED could have the lowest chance of not connecting.
This lets the hyperlaunched domU boot without stalling. Once dom0
starts xenconsoled, xl console can be used to access the domU's hvc0.
Paritally sync console.h from xen.git to bring in the new field.

MAX_HVC_IUCV_LINES == HVC_ALLOC_TTY_ADAPTERS == 8.
This is the number of entries in:
static struct hvc_iucv_private *hvc_iucv_table[MAX_HVC_IUCV_LINES];
Sometimes hvc_iucv_table[] is limited by:
(a) if (num > hvc_iucv_devices) // for error detection
or
(b) for (i = 0; i < hvc_iucv_devices; i++) // in 2 places
(so these 2 don't agree; second one appears to be correct to me.)
hvc_iucv_devices can be 0..8. This is a counter.
(c) if (hvc_iucv_devices > MAX_HVC_IUCV_LINES)
If hvc_iucv_devices == 8, (a) allows the code to access hvc_iucv_table[8].
Oops.

This is the exact same thing as the 'alloc_obj()' version, only much
smaller because there are a lot fewer users of the *alloc_flex()
interface.
As with alloc_obj() version, this was done entirely with mindless brute
force, using the same script, except using 'flex' in the pattern rather
than 'objs*'.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

Xen can support console_io hypercalls for any domains, not just dom0,
depending on DEBUG and XSM policies. These hypercalls can be very useful
for development and debugging.
Introduce a kernel command line option xen_console_io to enable the
usage of console_io hypercalls for any domain upon request. When
xen_console_io is not specified, the current behavior is retained.

The continual trickle of small conversion patches is grating on me, and
is really not helping. Just get rid of the 'remove_new' member
function, which is just an alias for the plain 'remove', and had a
comment to that effect:

* .remove_new() is a relic from a prototype conversion of .remove().
* New drivers are supposed to implement .remove(). Once all drivers are
* converted to not use .remove_new any more, it will be dropped.
*/
This was just a tree-wide 'sed' script that replaced '.remove_new' with
'.remove', with some care taken to turn a subsequent tab into two tabs
to make things line up.
I did do some minimal manual whitespace adjustment for places that used
spaces to line things up.
Then I just removed the old (sic) .remove_new member function, and this
is the end result. No more unnecessary conversion noise.

Refer ARM DDI 0487I.a ID081822, D17.3.8, DBGDTRTX_EL0,
"If TXfull is set to 1, set DTRRX and DTRTX to UNKNOWN"
Thus one should always check for TXfull condition before hvc can be
used as an early console. This is similar to what is being done
today in hvc_dcc_console_init() and hvc_dcc_init().
The count 0x4000000 has been obtained from uboot (v2024.01-rc3)
drivers/serial/arm_dcc.c "TIMEOUT_COUNT".
It reads the dcc status and waits for 0x4000000 times for the TX Fifo
to be available before returning an error. Thus, it will prevent DCC
to be used as early console.

This means:
* move outbuf to the end of struct hvc_struct and convert from pointer
to flexible array (the structure is smaller now)
* use struct_size() at the allocation site
* align outbuf in the struct instead of ALIGN() at kzalloc()
And apart from the above, use u8 instead of char (which are the same
thanks to -funsigned-char). The former is now preferred over the latter.
It makes the code easier to understand.
