PCI: Make reset_subordinate hotplug safe
PCI: Allow all bus devices to use the same slot
PCI: Rename __pci_bus_reset() and __pci_slot_reset()
A PCIe hotplug slot applies to the entire secondary bus. Thus, pciehp only
allocates a single hotplug_slot for the bridge to that bus. The existing
PCI slot, though, would only match to functions on device 0, meaning any
devices beyond that, e.g., ARI functions, are not matched to any slot even
though they share it. A slot reset will break all the missing devices
because the handling skips them.
For example, ARI devices with more than 8 functions fail because their
state is not properly handled, nor is the attached driver notified of the
reset. In the best case, the device will appear unresponsive to the driver,
resulting in unexpected errors. A worse possibility may panic the kernel if
in-flight transactions trigger hardware reported errors like this real
observation:

Allow a slot to be created to claim all devices on a bus, not just a
matching device. This is done by introducing a sentinel value, named
PCI_SLOT_ALL_DEVICES, which then has the PCI slot match to any device on
the bus. This fixes slot resets for pciehp.
Since 0xff already has special meaning, the chosen value for this new
feature is 0xfe. This will not clash with any actual slot number since they
are limited to 5 bits.

This converts some of the visually simpler cases that have been split
over multiple lines. I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.
Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script. I probably had made it a bit _too_ trivial.
So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.
The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

# Conflicts:

Currently work items enqueued by schedule_delayed_work() use "system_wq" (a
per-CPU wq), while queue_delayed_work() uses WORK_CPU_UNBOUND (used when a
CPU is not specified). The same applies to schedule_work() that is using
system_wq and queue_work(), that makes use again of WORK_CPU_UNBOUND. This
lack of consistency cannot be addressed without refactoring the API.
alloc_workqueue() treats all queues as per-CPU by default, while unbound
workqueues must opt-in via WQ_UNBOUND.
This default is suboptimal: most workloads benefit from unbound queues,
allowing the scheduler to place worker threads where they're needed and
reducing noise when CPUs are isolated.
This continues the effort to refactor workqueue APIs, which began with the
introduction of new workqueues and a new alloc_workqueue() flag in:
128ea9f6cc ("workqueue: Add system_percpu_wq and system_dfl_wq")
930c2ea566 ("workqueue: Add new WQ_PERCPU flag")
Add WQ_PERCPU to explicitly request alloc_workqueue() to be per-CPU when
WQ_UNBOUND has not been specified.
With the introduction of the WQ_PERCPU flag (equivalent to !WQ_UNBOUND),
any alloc_workqueue() caller that doesn't explicitly specify WQ_UNBOUND
must now use WQ_PERCPU.
Once migration is complete, WQ_UNBOUND can be removed and unbound will
become the implicit default.

Remove the macro in order to get rid of a pointless indirection. Replace
all users with the string it defines. In almost all cases this leads to a
simple replacement like this:
- #define KMSG_COMPONENT "appldata"
- #define pr_fmt(fmt) KMSG_COMPONENT ": " fmt

The PCIe hotplug driver calculates the depth of a nested hotplug port by
looking at the is_hotplug_bridge flag. The depth is used as lockdep class
to tell hotplug ports apart.
The is_hotplug_bridge flag encompasses ACPI slots handled by the ACPI
hotplug driver, hence the calculated depth may be too high. Avoid by
checking the is_pciehp flag instead.
This glitch likely has no user-visible impact: ACPI slots typically only
exist at the Root Port level, not in nested hotplug hierarchies. Also,
CONFIG_LOCKDEP is usually only used by developers. So this is just for
the sake of correctness.

The existing PowerNV hotplug code did not handle surprise plug events
correctly, leading to a complete failure of the hotplug system after device
removal and a required reboot to detect new devices.
This comes down to two issues:
1) When a device is surprise removed, often the bridge upstream
port will cause a PE freeze on the PHB. If this freeze is not
cleared, the MSI interrupts from the bridge hotplug notification
logic will not be received by the kernel, stalling all plug events
on all slots associated with the PE.
2) When a device is removed from a slot, regardless of surprise or
programmatic removal, the associated PHB/PE ls left frozen.
If this freeze is not cleared via a fundamental reset, skiboot
is unable to clear the freeze and cannot retrain / rescan the
slot. This also requires a reboot to clear the freeze and redetect
the device in the slot.
Issue the appropriate unfreeze and rescan commands on hotplug events,
and don't oops on hotplug if pci_bus_to_OF_node() returns NULL.

When the root of a nested PCIe bridge configuration is unplugged, the
pnv_php driver leaked the allocated IRQ resources for the child bridges'
hotplug event notifications, resulting in a panic.
Fix this by walking all child buses and deallocating all its IRQ resources
before calling pci_hp_remove_devices().
Also modify the lifetime of the workqueue at struct pnv_php_slot::wq so
that it is only destroyed in pnv_php_free_slot(), instead of
pnv_php_disable_irq(). This is required since pnv_php_disable_irq() will
now be called by workers triggered by hot unplug interrupts, so the
workqueue needs to stay allocated.

Commit c3be50f754 ("PCI: pciehp: Ignore Presence Detect Changed caused by
DPC") sought to ignore Presence Detect Changed events occurring as a side
effect of Downstream Port Containment.
The commit awaits recovery from DPC and then clears events which occurred
in the meantime. However if the first event seen after DPC is Data Link
Layer State Changed, only that event is cleared and not Presence Detect
Changed. The object of the commit is thus defeated.
That's because pciehp_ist() computes the events to clear based on the local
"events" variable instead of "ctrl->pending_events". The former contains
the events that had occurred when pciehp_ist() was entered, whereas the
latter also contains events that have accumulated while awaiting DPC
recovery.
In practice, the order of PDC and DLLSC events is arbitrary and the delay
in-between can be several milliseconds.
So change the logic to always clear PDC events, even if they come after an
