Benjamin MarzinskiandYu Kuai 418b3e64e4 md/raid5: Fix UAF on IO across the reshape position
If make_stripe_request() returns STRIPE_WAIT_RESHAPE,
raid5_make_request() will free the cloned bio. But raid5_make_request()
can call make_stripe_request() multiple times, writing to the various
stripes. If that bio got added to the toread or towrite lists of a
stripe disk in an earlier call to make_stripe_request(), then it's not
safe to just free the bio if a later part of it is found to cross the
reshape position. Doing so can lead to a UAF error, when bio_endio()
is called on the bio for the earlier stripes.

Instead, raid5_make_request() needs to wait until all parts of the bio
have called bio_endio(). To do this, bios that cross the reshape
position while the reshape can't make progress are flagged as needing to
wait for all parts to complete. When raid5_make_request() has a bio that
failed make_stripe_request() with STRIPE_WAIT_RESHAPE, it sets
bi->bi_private to a completion struct and waits for completion after
ending the bio.  When the bio_endio() is called for the last time on a
clone bio with bi->bi_private set, it wakes up the waiter. This
guarantees that raid5_make_request() doesn't return until the cloned bio
needing a retry for io across the reshape boundary is safely cleaned up.

There is a simple reproducer available at [1]. Compile the kernel with
KASAN for more useful reporting when the error is triggered (this is not
necessary to see the bug).

Chia-Ming ChangandYu Kuai 7f9f7c6974 md/raid5: fix soft lockup in retry_aligned_read()
When retry_aligned_read() encounters an overlapped stripe, it releases
the stripe via raid5_release_stripe() which puts it on the lockless
released_stripes llist. In the next raid5d loop iteration,
release_stripe_list() drains the stripe onto handle_list (since
STRIPE_HANDLE is set by the original IO), but retry_aligned_read()
runs before handle_active_stripes() and removes the stripe from
handle_list via find_get_stripe() -> list_del_init(). This prevents
handle_stripe() from ever processing the stripe to resolve the
overlap, causing an infinite loop and soft lockup.

Fix this by using __release_stripe() with temp_inactive_list instead
of raid5_release_stripe() in the failure path, so the stripe does not
go through the released_stripes llist. This allows raid5d to break out
of its loop, and the overlap will be resolved when the stripe is
eventually processed by handle_stripe().

FengWei ShihandYu Kuai 52e4324935 md/raid5: skip 2-failure compute when other disk is R5_LOCKED
When skip_copy is enabled on a doubly-degraded RAID6, a device that is
being written to will be in R5_LOCKED state with R5_UPTODATE cleared.
If a new read triggers fetch_block() while the write is still in
flight, the 2-failure compute path may select this locked device as a
compute target because it is not R5_UPTODATE.

Because skip_copy makes the device page point directly to the bio page,
reconstructing data into it might be risky. Also, since the compute
marks the device R5_UPTODATE, it triggers WARN_ON in ops_run_io()
which checks that R5_SkipCopy and R5_UPTODATE are not both set.

This can be reproduced by running small-range concurrent read/write on
a doubly-degraded RAID6 with skip_copy enabled, for example:

  mdadm -C /dev/md0 -l6 -n6 -R -f /dev/loop[0-3] missing missing
  echo 1 > /sys/block/md0/md/skip_copy
  fio --filename=/dev/md0 --rw=randrw --bs=4k --numjobs=8 \
      --iodepth=32 --size=4M --runtime=30 --time_based --direct=1

Fix by checking R5_LOCKED before proceeding with the compute. The
compute will be retried once the lock is cleared on IO completion.

Yu Kuai d51e1668fa md/raid5: set chunk_sectors to enable full stripe I/O splitting
Set chunk_sectors to the full stripe width (io_opt) so that the block
layer splits I/O at full stripe boundaries. This ensures that large
writes are aligned to full stripes, avoiding the read-modify-write
overhead that occurs with partial stripe writes in RAID-5/6.

When chunk_sectors is set, the block layer's bio splitting logic in
get_max_io_size() uses blk_boundary_sectors_left() to limit I/O size
to the boundary. This naturally aligns split bios to full stripe
boundaries, enabling more efficient full stripe writes.

Test results with 24-disk RAID5 (chunk_size=64k):
  dd if=/dev/zero of=/dev/md0 bs=10M oflag=direct
  Before: 461 MB/s
  After:  520 MB/s  (+12.8%)

Linus Torvalds 32a92f8c89 Convert more 'alloc_obj' cases to default GFP_KERNEL arguments
This converts some of the visually simpler cases that have been split
over multiple lines.  I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.

Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script.  I probably had made it a bit _too_ trivial.

So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.

The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

Linus Torvalds bf4afc53b7 Convert 'alloc_obj' family to use the new default GFP_KERNEL argument
This was done entirely with mindless brute force, using

    git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
        xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'

to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.

Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.

For the same reason the 'flex' versions will be done as a separate
conversion.

Kees Cook 69050f8d6d treewide: Replace kmalloc with kmalloc_obj for non-scalar types
This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

Yu Kuai cd1635d844 md/raid5: fix IO hang with degraded array with llbitmap
When llbitmap bit state is still unwritten, any new write should force
rcw, as bitmap_ops->blocks_synced() is checked in handle_stripe_dirtying().
However, later the same check is missing in need_this_block(), causing
stripe to deadloop during handling because handle_stripe() will decide
to go to handle_stripe_fill(), meanwhile need_this_block() always return
0 and nothing is handled.

Li NanandYu Kuai 5d1dd57929 md: remove recovery_disabled
'recovery_disabled' logic is complex and confusing, originally intended to
preserve raid in extreme scenarios. It was used in following cases:
- When sync fails and setting badblocks also fails, kick out non-In_sync
  rdev and block spare rdev from joining to preserve raid [1]
- When last backup is unavailable, prevent repeated add-remove of spares
  triggering recovery [2]

The original issues are now resolved:
- Error handlers in all raid types prevent last rdev from being kicked out
- Disks with failed recovery are marked Faulty and can't re-join

Therefore, remove 'recovery_disabled' as it's no longer needed.

Li NanandYu Kuai fd4d44c14f md: mark rdev Faulty when badblocks setting fails
Currently when sync read fails and badblocks set fails (exceeding
512 limit), rdev isn't immediately marked Faulty. Instead
'recovery_disabled' is set and non-In_sync rdevs are removed later.
This preserves array availability if bad regions aren't read, but bad
sectors might be read by users before rdev removal. This occurs due
to incorrect resync/recovery_offset updates that include these bad
sectors.

When badblocks exceed 512, keeping the disk provides little benefit
while adding complexity. Prompt disk replacement is more important.
Therefore when badblocks set fails, directly call md_error to mark rdev
Faulty immediately, preventing potential data access issues.

After this change, cleanup of offset update logic and 'recovery_disabled'
handling will follow.

Yu Kuai 4ffe28ed0d md/raid5: make sure max_sectors is not less than io_opt
Otherwise, even if user issue IO by io_opt, such IO will be split
by max_sectors before they are submitted to raid5. For consequence,
full stripe IO is impossible.

BTW, dm-raid5 is not affected and still have such problem.

Yu Kuai 9340a95d48 md/raid5: use mempool to allocate stripe_request_ctx
On the one hand, stripe_request_ctx is 72 bytes, and it's a bit huge for
a stack variable.

On the other hand, the bitmap sectors_to_do is a fixed size, result in
max_hw_sector_kb of raid5 array is at most 256 * 4k = 1Mb, and this will
make full stripe IO impossible for the array that chunk_size * data_disks
is bigger. Allocate ctx during runtime will make it possible to get rid
of this limit.

Yu Kuai 2d9f7150ac md/raid5: fix raid5_run() to return error when log_init() fails
Since commit f63f17350e ("md/raid5: use the atomic queue limit
update APIs"), the abort path in raid5_run() returns 'ret' instead of
-EIO. However, if log_init() fails, 'ret' is still 0 from the previous
successful call, causing raid5_run() to return success despite the
failure.

Fix this by capturing the return value from log_init().

Tuo LiandYu Kuai 7ad6ef91d8 md/raid5: fix possible null-pointer dereferences in raid5_store_group_thread_cnt()
The variable mddev->private is first assigned to conf and then checked:

  conf = mddev->private;
  if (!conf) ...

If conf is NULL, then mddev->private is also NULL. In this case,
null-pointer dereferences can occur when calling raid5_quiesce():

  raid5_quiesce(mddev, true);
  raid5_quiesce(mddev, false);

since mddev->private is assigned to conf again in raid5_quiesce(), and conf
is dereferenced in several places, for example:

  conf->quiesce = 0;
  wake_up(&conf->wait_for_quiescent);

To fix this issue, the function should unlock mddev and return before
invoking raid5_quiesce() when conf is NULL, following the existing pattern
in raid5_change_consistency_policy().

Following test can cause IO hang:

mdadm -CvR /dev/md0 -l10 -n4 /dev/sd[abcd] --assume-clean --chunk=64K --bitmap=none
sleep 5
echo 1 > /sys/block/sda/device/delete
echo 1 > /sys/block/sdb/device/delete
echo 1 > /sys/block/sdc/device/delete
echo 1 > /sys/block/sdd/device/delete

dd if=/dev/md0 of=/dev/null bs=8k count=1 iflag=direct

Root cause:

1) all disks removed, however all rdevs in the array is still in sync,
IO will be issued normally.

2) IO failure from sda, and set badblocks failed, sda will be faulty
and MD_SB_CHANGING_PENDING will be set.

3) error recovery try to recover this IO from other disks, IO will be
issued to sdb, sdc, and sdd.

4) IO failure from sdb, and set badblocks failed again, now array is
broken and will become read-only.

5) IO failure from sdc and sdd, however, stripe can't be handled anymore
because MD_SB_CHANGING_PENDING is set:

Li NanandYu Kuai 62ed1b5822 md: allow configuring logical block size
Previously, raid array used the maximum logical block size (LBS)
of all member disks. Adding a larger LBS disk at runtime could
unexpectedly increase RAID's LBS, risking corruption of existing
partitions. This can be reproduced by:

  # LBS of sd[de] is 512 bytes, sdf is 4096 bytes.
  mdadm -CRq /dev/md0 -l1 -n3 /dev/sd[de] missing --assume-clean

  # LBS is 512
  cat /sys/block/md0/queue/logical_block_size

  # create partition md0p1
  parted -s /dev/md0 mklabel gpt mkpart primary 1MiB 100%
  lsblk | grep md0p1

  # LBS becomes 4096 after adding sdf
  mdadm --add -q /dev/md0 /dev/sdf
  cat /sys/block/md0/queue/logical_block_size

  # partition lost
  partprobe /dev/md0
  lsblk | grep md0p1
```

Simply restricting larger-LBS disks is inflexible. In some scenarios,
only disks with 512 bytes LBS are available currently, but later, disks
with 4KB LBS may be added to the array.

Making LBS configurable is the best way to solve this scenario.
After this patch, the raid will:
  - store LBS in disk metadata
  - add a read-write sysfs 'mdX/logical_block_size'

Future mdadm should support setting LBS via metadata field during RAID
creation and the new sysfs. Though the kernel allows runtime LBS changes,
users should avoid modifying it after creating partitions or filesystems
to prevent compatibility issues.

Only 1.x metadata supports configurable LBS. 0.90 metadata inits all
fields to default values at auto-detect. Supporting 0.90 would require
more extensive changes and no such use case has been observed.

Note that many RAID paths rely on PAGE_SIZE alignment, including for
metadata I/O. A larger LBS than PAGE_SIZE will result in metadata
read/write failures. So this config should be prevented.

Zhang YiandYu Kuai f0bd03832f md: init queue_limits->max_hw_wzeroes_unmap_sectors parameter
The parameter max_hw_wzeroes_unmap_sectors in queue_limits should be
equal to max_write_zeroes_sectors if it is set to a non-zero value.
However, the stacked md drivers call md_init_stacking_limits() to
initialize this parameter to UINT_MAX but only adjust
max_write_zeroes_sectors when setting limits. Therefore, this
discrepancy triggers a value check failure in blk_validate_limits().

 $ modprobe scsi_debug num_parts=2 dev_size_mb=8 lbprz=1 lbpws=1
 $ mdadm --create /dev/md0 --level=0 --raid-device=2 /dev/sda1 /dev/sda2
   mdadm: Defaulting to version 1.2 metadata
   mdadm: RUN_ARRAY failed: Invalid argument

Fix this failure by explicitly setting max_hw_wzeroes_unmap_sectors to
max_write_zeroes_sectors. Since the linear and raid0 drivers support
write zeroes, so they can support unmap write zeroes operation if all of
the backend devices support it. However, the raid1/10/5 drivers don't
support write zeroes, so we have to set it to zero.

If bio is split by internal handling like chunksize or badblocks, the
corresponding trace_block_split() is missing, resulting in blktrace
inability to catch BIO split events and making it harder to analyze the
BIO sequence.
