Linus Torvalds bf4afc53b7 Convert 'alloc_obj' family to use the new default GFP_KERNEL argument
This was done entirely with mindless brute force, using

    git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
        xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'

to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.

Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.

For the same reason the 'flex' versions will be done as a separate
conversion.

Kees Cook 69050f8d6d treewide: Replace kmalloc with kmalloc_obj for non-scalar types
This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

__blkdev_issue_discard() always returns 0, making all error checking
at call sites dead code.

For dm-thin change issue_discard() return type to void, in
passdown_double_checking_shared_status() remove the r assignment from
return value of the issue_discard(), for end_discard() hardcode value of
r to 0 that matches only value returned from __blkdev_issue_discard().

Benjamin MarzinskiandMikulas Patocka be4addb191 dm: Fix deadlock when reloading a multipath table
Request-based devices (dm-multipath) queue I/O in blk-mq on noflush
suspends. Any queued IO will make it impossible to freeze the queue. If
a process attempts to update the queue limits while there is queued IO,
it can be get stuck holding the limits lock, while unable to freeze the
queue. If device-mapper then attempts to update the limits during a
table swap, it will deadlock trying to grab the limits lock while making
it impossible to flush the IO.

Disallow updating the queue limits during a table swap, when updating an
immutable request-based dm device (dm-multipath) during a noflush
suspend. It is userspace's responsibility to make sure that the new
table uses the same limits as the existing table if it asks for a
noflush suspend.

Krister JohansenandMikulas Patocka 80f130bfad dm thin: make get_first_thin use rcu-safe list first function
The documentation in rculist.h explains the absence of list_empty_rcu()
and cautions programmers against relying on a list_empty() ->
list_first() sequence in RCU safe code.  This is because each of these
functions performs its own READ_ONCE() of the list head.  This can lead
to a situation where the list_empty() sees a valid list entry, but the
subsequent list_first() sees a different view of list head state after a
modification.

In the case of dm-thin, this author had a production box crash from a GP
fault in the process_deferred_bios path.  This function saw a valid list
head in get_first_thin() but when it subsequently dereferenced that and
turned it into a thin_c, it got the inside of the struct pool, since the
list was now empty and referring to itself.  The kernel on which this
occurred printed both a warning about a refcount_t being saturated, and
a UBSAN error for an out-of-bounds cpuid access in the queued spinlock,
prior to the fault itself.  When the resulting kdump was examined, it
was possible to see another thread patiently waiting in thin_dtr's
synchronize_rcu.

The thin_dtr call managed to pull the thin_c out of the active thins
list (and have it be the last entry in the active_thins list) at just
the wrong moment which lead to this crash.

Fortunately, the fix here is straight forward.  Switch get_first_thin()
function to use list_first_or_null_rcu() which performs just a single
READ_ONCE() and returns NULL if the list is already empty.

This was run against the devicemapper test suite's thin-provisioning
suites for delete and suspend and no regressions were observed.

Remove use of the blk_limits_io_{min,opt} and assign the values directly
to the queue_limits structure.  For the io_opt this is a completely
mechanical change, for io_min it removes flooring the limit to the
physical and logical block size in the particular caller.  But as
blk_validate_limits will do the same later when actually applying the
limits, there still is no change in overall behavior.

Mike Snitzer 825d8bbd2f dm: always manage discard support in terms of max_hw_discard_sectors
Commit 4f563a6473 ("block: add a max_user_discard_sectors queue
limit") changed block core to set max_discard_sectors to:
 min(lim->max_hw_discard_sectors, lim->max_user_discard_sectors)

Since commit 1c0e720228 ("dm: use queue_limits_set") it was reported
dm-thinp was failing in a few fstests (generic/347 and generic/405)
with the first WARN_ON_ONCE in dm_cell_key_has_valid_range() being
reported, e.g.:
WARNING: CPU: 1 PID: 30 at drivers/md/dm-bio-prison-v1.c:128 dm_cell_key_has_valid_range+0x3d/0x50

blk_set_stacking_limits() sets max_user_discard_sectors to UINT_MAX,
so given how block core now sets max_discard_sectors (detailed above)
it follows that blk_stack_limits() stacks up the underlying device's
max_hw_discard_sectors and max_discard_sectors is set to match it. If
max_hw_discard_sectors exceeds dm's BIO_PRISON_MAX_RANGE, then
dm_cell_key_has_valid_range() will trigger the warning with:
WARN_ON_ONCE(key->block_end - key->block_begin > BIO_PRISON_MAX_RANGE)

Aside from this warning, the discard will fail.  Fix this and other DM
issues by governing discard support in terms of max_hw_discard_sectors
instead of max_discard_sectors.

Mike Snitzer ef6953fb68 dm thin: update .io_hints methods to not require handling discards last
Removes assumptions about what might follow the discard setup code
(previously the code would return early if discards not enabled).

Makes it possible to add more capabilites to the end of each .io_hints
method (which is the natural thing to do when adding new features).

Mike Snitzer 722d908223 dm thin: fix issue_discard to pass GFP_NOIO to __blkdev_issue_discard
issue_discard() passes GFP_NOWAIT to __blkdev_issue_discard() despite
its code assuming bio_alloc() always succeeds.

Commit 3dba53a958 ("dm thin: use __blkdev_issue_discard for async
discard support") clearly shows where things went bad:

Before commit 3dba53a958, dm-thin.c's open-coded
__blkdev_issue_discard_async() properly handled using GFP_NOWAIT.
Unfortunately __blkdev_issue_discard() doesn't and it was missed
during review.

Christoph HellwigandJens Axboe 05bdb99653 block: replace fmode_t with a block-specific type for block open flags
The only overlap between the block open flags mapped into the fmode_t and
other uses of fmode_t are FMODE_READ and FMODE_WRITE.  Define a new
blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and
->ioctl and stop abusing fmode_t.
