mm: cache struct page for empty_zero_page and return it from ZERO_PAGE()
For most architectures every invocation of ZERO_PAGE() does virt_to_page(empty_zero_page). But empty_zero_page is in BSS and it is enough to get its struct page once at initialization time and then use it whenever a zero page should be accessed. Add yet another __zero_page variable that will be initialized as virt_to_page(empty_zero_page) for most architectures in a weak arch_setup_zero_pages() function. For architectures that use colored zero pages (MIPS and s390) rename their setup_zero_pages() to arch_setup_zero_pages() and make it global rather than static. For architectures that cannot use virt_to_page() for BSS (arm64 and sparc64) add override of arch_setup_zero_pages(). MIPS: mm: Rewrite TLB uniquification for the hidden bit feature
Before the introduction of the EHINV feature, which lets software mark
TLB entries invalid, certain older implementations of the MIPS ISA were
equipped with an analogous bit, as a vendor extension, which however is
hidden from software and only ever set at reset, and then any software
write clears it, making the intended TLB entry valid.
This feature makes it unsafe to read a TLB entry with TLBR, modify the
page mask, and write the entry back with TLBWI, because this operation
will implicitly clear the hidden bit and this may create a duplicate
entry, as with the presence of the hidden bit there is no guarantee all
the entries across the TLB are unique each.
Usually the firmware has already uniquified TLB entries before handing
control over, in which case we only need to guarantee at bootstrap no
clash will happen with the VPN2 values chosen in local_flush_tlb_all().
However with systems such as Mikrotik RB532 we get handed the TLB as at
reset, with the hidden bit set across the entries and possibly duplicate
entries present. This then causes a machine check exception when page
sizes are reset in r4k_tlb_uniquify() and prevents the system from
booting.
Rewrite the algorithm used in r4k_tlb_uniquify() then such as to avoid
the reuse of ASID/VPN values across the TLB. Get rid of global entries
first as they may be blocking the entire address space, e.g. 16 256MiB
pages will exhaust the whole address space of a 32-bit CPU and a single
big page can exhaust the 32-bit compatibility space on a 64-bit CPU.
Details of the algorithm chosen are given across the code itself.

MIPS: mm: Suppress TLB uniquification on EHINV hardware
Hardware that supports the EHINV feature, mandatory for R6 ISA and FTLB implementation, lets software mark TLB entries invalid, which eliminates the need to ensure no duplicate matching entries are ever created. This feature is already used by local_flush_tlb_all(), via the UNIQUE_ENTRYHI macro, making the preceding call to r4k_tlb_uniquify() superfluous. The next change will also modify uniquification code such that it'll become incompatible with the FTLB and MMID features, as well as MIPSr6 CPUs that do not implement 4KiB pages. Therefore prevent r4k_tlb_uniquify() from being used on EHINV hardware, as denoted by `cpu_has_tlbinv'. Signed-off-by: Maciej W. MIPS: SiByte: Bring back cache initialisation
Bring back cache initialisation for Broadcom SiByte SB1 cores, which has
been removed causing the kernel to hang at bootstrap right after:
Dentry cache hash table entries: 524288 (order: 8, 4194304 bytes, linear)
Inode-cache hash table entries: 262144 (order: 7, 2097152 bytes, linear)
The cause of the problem is R4k cache handlers are also used by Broadcom
SiByte SB1 cores, however with a different cache error exception handler
and therefore not using CPU_R4K_CACHE_TLB:

arch, mm: consolidate initialization of nodes, zones and memory map
To initialize node, zone and memory map data structures every architecture calls free_area_init() during setup_arch() and passes it an array of zone limits. Beside code duplication it creates "interesting" ordering cases between allocation and initialization of hugetlb and the memory map. Some architectures allocate hugetlb pages very early in setup_arch() in certain cases, some only create hugetlb CMA areas in setup_arch() and sometimes hugetlb allocations happen mm_core_init(). With arch_zone_limits_init() helper available now on all architectures it is no longer necessary to call free_area_init() from architecture setup code. Rather core MM initialization can call arch_zone_limits_init() in a single place. This allows to unify ordering of hugetlb vs memory map allocation and initialization. Remove the call to free_area_init() from architecture specific code and place it in a new mm_core_init_early() function that is called immediately after setup_arch(). After this refactoring it is possible to consolidate hugetlb allocations and eliminate differences in ordering of hugetlb and memory map initialization among different architectures. As the first step of this consolidation move hugetlb_bootmem_alloc() to mm_core_early_init(). MIPS: mm: Prevent a TLB shutdown on initial uniquification
Depending on the particular CPU implementation a TLB shutdown may occur
if multiple matching entries are detected upon the execution of a TLBP
or the TLBWI/TLBWR instructions. Given that we don't know what entries
we have been handed we need to be very careful with the initial TLB
setup and avoid all these instructions.
Therefore read all the TLB entries one by one with the TLBR instruction,
bypassing the content addressing logic, and truncate any large pages in
place so as to avoid a case in the second step where an incoming entry
for a large page at a lower address overlaps with a replacement entry
chosen at another index. Then preinitialize the TLB using addresses
outside our usual unique range and avoiding clashes with any entries
received, before making the usual call to local_flush_tlb_all().
This fixes (at least) R4x00 cores if TLBP hits multiple matching TLB
entries (SGI IP22 PROM for examples sets up all TLBs to the same virtual
address).

Let's make it clearer that we are operating within a single folio by providing both the folio and the page. This implies that for flush_dcache_folio() we'll now avoid one more page->folio lookup, and that we can safely drop the "nth_page" usage. While at it, drop the "extern" from the function declaration. MIPS: mm: tlb-r4k: Uniquify TLB entries on init
Hardware or bootloader will initialize TLB entries to any value, which may collide with kernel's UNIQUE_ENTRYHI value. On MIPS microAptiv/M5150 family of cores this will trigger machine check exception and cause boot failure. On M5150 simulation this could happen 7 times out of 1000 boots. Replace local_flush_tlb_all() with r4k_tlb_uniquify() which probes each TLB ENTRIHI unique value for collisions before it's written, and in case of collision try a different ASID. The point where the memory is released from memblock to the buddy allocator is hidden inside arch-specific mem_init()s and the call to memblock_free_all() is needlessly duplicated in every artiste cure and after introduction of arch_mm_preinit() hook, mem_init() implementation on many architecture only contains the call to memblock_free_all(). Pull memblock_free_all() call into mm_core_init() and drop mem_init() on relevant architectures to make it more explicit where the free memory is released from memblock to the buddy allocator and to reduce code duplication in architecture specific code. Currently, implementation of mem_init() in every architecture consists of one or more of the following: * initializations that must run before page allocator is active, for instance swiotlb_init() * a call to memblock_free_all() to release all the memory to the buddy allocator * initializations that must run after page allocator is ready and there is no arch-specific hook other than mem_init() for that, like for example register_page_bootmem_info() in x86 and sparc64 or simple setting of mem_init_done = 1 in several architectures * a bunch of semi-related stuff that apparently had no better place to live, for example a ton of BUILD_BUG_ON()s in parisc. Introduce arch_mm_preinit() that will be the first thing called from mm_core_init(). On architectures that have initializations that must happen before the page allocator is ready, move those into arch_mm_preinit() along with the code that does not depend on ordering with page allocator setup. On several architectures this results in reduction of mem_init() to a single call to memblock_free_all() that allows its consolidation next. All architectures that support HIGHMEM have their code that frees high memory pages to the buddy allocator while __free_memory_core() is limited to freeing only low memory. There is no actual reason for that. The memory map is completely ready by the time memblock_free_all() is called and high pages can be released to the buddy allocator along with low memory. Remove low memory limit from __free_memory_core() and drop per-architecture code that frees high memory pages. high_memory defines upper bound on the directly mapped memory. This bound is defined by the beginning of ZONE_HIGHMEM when a system has high memory and by the end of memory otherwise. All this is known to generic memory management initialization code that can set high_memory while initializing core mm structures. Add a generic calculation of high_memory to free_area_init() and remove per-architecture calculation except for the architectures that set and use high_memory earlier than that. max_mapnr is essentially the size of the memory map for systems that use FLATMEM. There is no reason to calculate it in each and every architecture when it's anyway calculated in alloc_node_mem_map(). Drop setting of max_mapnr from architecture code and set it once in alloc_node_mem_map(). mm/ioremap: pass pgprot_t to ioremap_prot() instead of unsigned long
ioremap_prot() currently accepts pgprot_val parameter as an unsigned long, thus implicitly assuming that pgprot_val and pgprot_t could never be bigger than unsigned long. But this assumption soon will not be true on arm64 when using D128 pgtables. In 128 bit page table configuration, unsigned long is 64 bit, but pgprot_t is 128 bit. Passing platform abstracted pgprot_t argument is better as compared to size based data types. Let's change the parameter to directly pass pgprot_t like another similar helper generic_ioremap_prot(). Without this change in place, D128 configuration does not work on arm64 as the top 64 bits gets silently stripped when passing the protection value to this function. asm-generic: pgalloc: provide generic __pgd_{alloc,free}
We already have a generic implementation of alloc/free up to P4D level, as
well as pgd_free(). Let's finish the work and add a generic PGD-level
alloc helper as well.
Unlike at lower levels, almost all architectures need some specific magic
at PGD level (typically initialising PGD entries), so introducing a
generic pgd_alloc() isn't worth it. Instead we introduce two new helpers,
__pgd_alloc() and __pgd_free(), and make use of them in the arch-specific
pgd_alloc() and pgd_free() wherever possible. To accommodate as many arch
as possible, __pgd_alloc() takes a page allocation order.
Because pagetable_alloc() allocates zeroed pages, explicit zeroing in
pgd_alloc() becomes redundant and we can get rid of it. Some trivial
implementations of pgd_free() also become unnecessary once __pgd_alloc()
is used; remove them.
Another small improvement is consistent accounting of PGD pages by using
GFP_PGTABLE_{USER,KERNEL} as appropriate.
Not all PGD allocations can be handled by the generic helpers. In
particular, multiple architectures allocate PGDs from a kmem_cache, and
those PGDs may not be page-sized.
