Make ArcGetMemoryDescriptor() static since it's only needed internally.

Add a cfe_die() implementation which is useful when the kernel does an
early panic and no console is registered. This allows us to print
useful diagnostics such as an invalid DTB having been
configured/selected.
Since the BMIPS_GENERIC kernel can be built with support for multiple
processors, we need to do a runtime determination of the type of CPU
that we are executing on to perform the appropriate XKS01 disabling.
Since cfe_init() + cfe_die() could be conceivably called at very early
stages of the kernel boot, before cpu_probe(), we do not rely on the
structure(s) populated by cpu_probe().

Most of the prom_free_prom_memory functions are empty. With
a new weak prom_free_prom_memory() we can remove all of them.

add_memory_region was the old interface for registering memory and
was already changed to used memblock internaly. Replace it by
directly calling memblock functions.

As far as I understood, prom_meminit() in arch/mips/fw/arc/memory.c
is overridden by the one in arch/mips/sgi-ip32/ip32-memory.c if
CONFIG_SGI_IP32 is enabled.
The use of EXPORT_SYMBOL in static libraries potentially causes a
problem for the llvm linker [1]. So, I want to forcibly link lib-y
objects to vmlinux when CONFIG_MODULES=y.
As a groundwork, we must fix multiple definitions that have previously
been hidden by lib-y.
The prom_cleanup() in this file is already marked as __weak (because
it is overridden by the one in arch/mips/sgi-ip22/ip22-mc.c).
I think it should be OK to do the same for these two.

MIPS provides multiple definitions for the following functions:
fw_init_cmdline
__delay
__udelay
__ndelay
memmove
__rmemcpy
memcpy
__copy_user
The generic ones are defined in lib-y objects, which are overridden by
the Octeon ones when CONFIG_CAVIUM_OCTEON_SOC is enabled.

If you look at lib/string.c, arch can define __HAVE_ARCH_* to opt out
the generic implementation.
Similarly, this commit adds CONFIG_HAVE_PLAT_* to allow a platform
to opt out the MIPS generic code.

Pull in mips-fixes primarily to gain build fixes in order to allow
better testing of mips-next.
A few MIPS fixes:
- Fix VDSO time-related function behavior for systems where we need to
fall back to syscalls, but were instead returning bogus results.
- A fix to TLB exception handlers for Cavium Octeon systems where they
would inadvertently clobber the $1/$at register.
- A build fix for bcm63xx configurations.
- Switch to using my @kernel.org email address.

EARLY_PRINTK uses ArcWrite (via prom_putchar) on IP22/28, which needs
to not mess up PROMs data structures. ARC PROM gives out a list of
memory chunks, which are used and which are free. This fixes the
problem of not working early printk.
By using XKPHYS spaces more than 256MB memory on Indigo2 R4k machines
is working now, too.

Pointer arguments for 32bit ARC PROMs must reside in CKSEG0/1. While
the initial stack resides in CKSEG0 the first kernel thread stack
is already placed at a XKPHYS address, which ARC32 can't handle.
The workaround here is to use static variables, which are placed
into BSS and linked to a CKSEG0 address.
