migrate_add_blocker_modes() and migration_add_notifier_modes use
variable arguments for a set of migration modes.  The variable
arguments get collected into a bitset for processsing.  Take a bitset
argument instead, it's simpler.

John LevonandCédric Le Goater ecbe424a63 vfio: only check region info cache for initial regions 
It is semantically valid for a VFIO device to increase the number of
regions after initialization. In this case, we'd attempt to check for
cached region info past the size of the ->reginfo array. Check for the
region index and skip the cache in these cases.

This also works around some VGPU use cases which appear to be a bug,
where VFIO_DEVICE_QUERY_GFX_PLANE returns a region index beyond the
reported ->num_regions.

We set VFIODevice::num_regions at initialization time, and do not
otherwise refresh it. As it is valid in theory for a VFIO device to
later increase the number of supported regions, rename the field to
"num_initial_regions" to better reflect its semantics.

Steve SistareandCédric Le Goater b9b389b9e0 vfio/iommufd: invariant device name 
cpr-transfer will use the device name as a key to find the value
of the device descriptor in new QEMU.  However, if the descriptor
number is specified by a command-line fd parameter, then
vfio_device_get_name creates a name that includes the fd number.
This causes a chicken-and-egg problem: new QEMU must know the fd
number to construct a name to find the fd number.

To fix, create an invariant name based on the id command-line parameter,
if id is defined.  The user will need to provide such an id to use CPR.

Split out parts of TYPE_VFIO_PCI into a base TYPE_VFIO_PCI_BASE,
although we have not yet introduced another subclass, so all the
properties have remained in TYPE_VFIO_PCI.

Note that currently there is no need for additional data for
TYPE_VFIO_PCI, so it shares the same C struct type as
TYPE_VFIO_PCI_BASE, VFIOPCIDevice.

Instead of requesting region information on demand with
VFIO_DEVICE_GET_REGION_INFO, maintain a cache: this will become
necessary for performance for vfio-user, where this call becomes a
message over the control socket, so is of higher overhead than the
traditional path.

We will also need it to generalize region accesses, as that means we
can't use ->config_offset for configuration space accesses, but must
look up the region offset (if relevant) each time.
