Michael Maltese 04158dfe15 AVIDump: use a separate AVCodecContext 
Using the AVCodecContext contained in AVStream for muxing is officially
discouraged[1] and AVStream::codec was deprecated in favor of
AVStream::codecpar in libavformat 57.33.100 / 57.5.0.
