725bf9dcaf phy-rcar-usb: correct base address
The memory region that is used by the driver overlaps EHCI and OHCI  register
regions for absolutely no reason now  -- fix it  by adding offset of 0x800 to
the base address, changing the register #define's accordingly. This has extra
positive effect that we now can use devm_ioremap_resource()...

Note that the driver and the SoC code have to be in one patch to keep the code
bisectable...

The patch has been tested on the Marzen board.

463c824bb7 phy-rcar-usb: remove EHCI internal buffer setup
Now that the EHCI internal  buffer setup is done by the platform code,  we  can
remove  such code from this driver as it  never  really belonged here.  We also
no longer need the 2nd memory region now (2nd EHCI controller is simply missing
in e.g. R8A7778 SoC).

The patch has been tested on the Marzen and BOCK-W boards.

743fcce0a8 ehci-platform: add pre_setup() method to platform data
Sometimes there is a need  to initialize some non-standard registers mapped to
the EHCI region before accessing the standard EHCI registers.  Add pre_setup()
method with 'struct usb_hcd *' parameter to be called just before ehci_setup()
to the 'ehci-platform'  driver's platform data for this purpose...

While at it, add the missing incomplete declaration of 'struct platform_device'
to <linux/usb/ehci_pdriver.h>...

The patch has been tested on the Marzen and BOCK-W boards.

0460fef2a9 aio: use cancellation list lazily
Cancelling kiocbs requires adding them to a per kioctx linked list,
which is one of the few things we need to take the kioctx lock for in
the fast path.  But most kiocbs can't be cancelled - so if we just do
this lazily, we can avoid quite a bit of locking overhead.

While we're at it, instead of using a flag bit switch to using ki_cancel
itself to indicate that a kiocb has been cancelled/completed.  This lets
us get rid of ki_flags entirely.

a80bf61ef3 gadget: remove only user of aio retry
This removes the only in-tree user of aio retry.  This will let us
remove the retry code from the aio core.

Removing retry is relatively easy as the USB gadget wasn't using it to
retry IOs at all.  It always fully submitted the IO in the context of
the initial io_submit() call.  It only used the AIO retry facility to
get the submitter's mm context for copying the result of a read back to
user space.  This is easy to implement with use_mm() and a work struct,
much like kvm does with async_pf_execute() for get_user_pages().

Fixed conflict in kernel/rtmutex-tester.c, the locking tree had a better
fix for the same sysfs file mode problem.
