Linas Vepstas 8d63f37505 [SCSI] lpfc: add PCI error recovery support ...

This patch adds PCI Error recovery support to the
Emulex Lightpulse Fibrechannel (lpfc) SCSI device driver.
Lightly tested at this point, works.
