Use vm_iomap_memory() instead of [io_]remap_pfn_range().
vm_iomap_memory() gives us much simpler API to map memory to userspace,
and reduces possibilities for bugs.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Since commit [e58aa3d2: genirq: Run irq handlers with interrupts disabled],
We run all interrupt handlers with interrupts disabled
and we even check and yell when an interrupt handler
returns with interrupts enabled (see commit [b738a50a:
genirq: Warn when handler enables interrupts]).
So now this flag is a NOOP and can be removed.

The -rt patches change the console_semaphore to console_mutex. As a
result, a quite large chunk of the patches changes all
acquire/release_console_sem() to acquire/release_console_mutex()
This commit makes things use more neutral function names which dont make
implications about the underlying lock.
The only real change is the return value of console_trylock which is
inverted from try_acquire_console_sem()
This patch also paves the way to switching console_sem from a semaphore to
a mutex.

During cleanup, use L1GPU_CONTEXT_ATTRIBUTE_FB_CLOSE to tear down the setup
done by L1GPU_CONTEXT_ATTRIBUTE_FB_SETUP.
This allows unloading and reloading of ps3fb while the sound driver keeps the
GPU open.

- Make the IOMMU flags used for mapping main memory into the GPU's I/O space
explicit, instead of relying on the default in the hypervisor,
- Add missing calls to lv1_gpu_context_iomap(..., CBE_IOPTE_M) to unmap the
memory during cleanup.

commit 059e4938f8 ("powerpc/ps3: Add a sub-match
id to ps3_system_bus") forgot to update the module alias support:
- Add the sub-match ids to the module aliases, so udev can distinguish
between different types of sub-devices.
- Rename PS3_MODULE_ALIAS_GRAPHICS to PS3_MODULE_ALIAS_GPU_FB, as ps3fb
binds to the "FB" sub-device.

Introduce ps3_gpu_mutex to synchronizes GPU-related operations, like:
- invoking the L1GPU_CONTEXT_ATTRIBUTE_FB_BLIT command using the
lv1_gpu_context_attribute() hypervisor call,
- handling the PS3AV_CID_AVB_PARAM packet in the PS3 A/V Settings driver.

cleanup sweep:
- Kill ps3fb_priv.xdr_ea and ps3fb_priv.xdr_size, use info->screen_base and
info->fix.smem_len instead.
- Kill superfluous assignments to info->fix.smem_start, info->fix.smem_len,
and info->screen_base in ps3fb_set_par(). Their values never change.
- Add sparse annotations to casts to kill address space warnings
