[PATCH] ARM: cleanup vmalloc start/offset macros

Browse Source 
VMALLOC_START and VMALLOC_OFFSET are common between all ARM
machine classes.  Move them into include/asm-arm/pgtable.h,
but allow a machine class to override them if required.
