Fix nearly all compilation warnings under Apple gcc-4.0. Tested with OPT="-g
-Wall -Wstrict-prototypes -Werror" in both --with-pydebug mode and --without.
There's still a batch of non-prototype warnings in Xlib.h that I don't know how
to fix.

#2016 Fix a crash in function call when the **kwargs dictionary is mutated
during the function call setup.
This even gives a slight speedup, probably because tuple allocation
is faster than PyMem_NEW.

module, and remove associated code from test_struct. This was a
mechanism for skipping some of the tests for overflow behaviour when
packing integers; it's no longer necessary.
when packing an integer: for example, struct.pack('=L', -1) now
raises struct.error instead of returning b'\xff\xff\xff\xff'.
Thanks Andreas Schawo for the patch.
nb_long: remove last remaining use of nb_long
(in the struct module) from the core, set nb_long slots on all builtin
and extension types to 0, and remove uses of __long__ in test_complex
and test_binop.
